Must have active operating authority registered with the FMCSA.
Certificate of Insurance (COI)
Minimum $1,000,000 Auto Liability and $100,000 Cargo Insurance naming MAB Dispatchers as certificate holder.
Signed W-9 Form
Current signed and dated W-9 matching your corporate or sole proprietorship filing.
Factoring Notice of Assignment (NOA)
Official bank remittance instructions from your factoring company (or voided check if self-invoicing).
